Illinois Diabetes Prevention and Control Program
The Coles County Health Department offers a free,
informational based
program that
will apprise participants of issues relevant to persons living
with
diabetes and services available to them. Participants receive a
bi-monthly newsletter and updates on available services. The purpose of
this program is to identify persons with
diabetes and to provide informational ,
educational newsletters that
include such topics as medication, foot/eye
care, recipes and local
resources.

217-348-0530 for additional information. To register for the IDPC
program, please
download and print the
Initial Form and
the
Cornerstone Consent Form. Completely
fill out the forms and mail them to the Coles County Health Department,
Attn: Misti Farler,
825 18th Street, Charleston, IL 61920. Once the forms are received,
you will be enrolled
in this FREE, informational based program.
