New Certificate

The Illinois Food Service Sanitation Manager Certification certificate is valid for five years. Your new 5-year certificate will be issued after the $35 fee has been received.

Certificate Renewal

If your certificate expires within the next two months and you have not received a renewal notice, submit this form to the Illinois Department of Public Health. A new, five-year certificate will be issued after a $35 fee has been received. It is very important that your request for a renewed certificate be received four weeks prior to the expiration date.

Replacement/Duplicate Certificate

If you need to replace a lost, stolen or misplaced certificate, please complete this form. A duplicate certificate, with your existing expiration date, will be issued after a $10 fee has been received.

Name and/or Address Change (Need New Certificate)

If you have changed your name (documentation of name changes are necessary) or have an address change, and you need a new certificate which shows these changes, please complete and submit this form. You will receive an updated certificate after a $10 fee has been received. The certificate holder is required by law to notify the Department of any address changes.

Name and Address Changes (Notification Only-New Certificate Not Needed)

If you have changed your name (documentation of name changes are necessary) or have an address change, but you do not need a new certificate which shows these changes, please complete and submit this form. There is no fee for changes if you do not need an updated certificate. You must notify the Department of address changes to ensure that you receive your renewal application prior to the expiration date.

Reciprocity

If you have a current Food Service Sanitation Manager Certification Certificate (Only City of Chicago Certificates accepted at this time) and you wish to receive an Illinois certificate please complete and submit this form along with a photocopy of your Food Service Sanitation Manager Certification Certificate. Our procedures require that we must first verify that you have a valid Food Service Sanitation Manager Certification Certificate. When we receive this verification, we will send you an application. Do NOT send any money with this request. You will be billed. Please allow 6 - 9 weeks for processing.
